Determination of Eligibility
A student requesting the Optional Practical Training (OPT) benefit must obtain a report of their grades to date of all courses in their final semester in order to determine whether the student may be eligible to apply for work authorization contingent on their F-1 student visa status. If a student is unable to obtain a written signature from their professors, an email with the relevant information from the professor’s HPU email address may suffice.

If you have not gotten in-person signatures from your professors or faculty/academic advisors, please have them send the requested information through email to iss@hpu.edu.
Even if you've submitted your professors and advisors information through email, you must still submit the information requested in Section A (your requested dates).

- Payable to "The U.S. Department of Homeland Security"
- Write your I-94 number and date of birth on the memo portion of the check/money order
- Check must have an imprinted or handwritten name and address in the upper left corner

- At the top, mark: Initial permission to accept employment
- Item 27: Eligibility Category is (c) (3) (B) [small c, number 3, capital B]
- Signature: Use black ink to sign name on the signature line, keep signature within the box
- Ensure that all date fields are per the required format (mm/dd/yyyy)
- Print your document as a PDF. Saving it as a PDF may not suffice!

- Lightly print your name and I-94 number on the back of each photo
- Passport photos must be 2" x 2" with proper facial dimensions of U.S. passport-style photos
