Change of Catalog Term Request

Complete this form to request to change your catalog term. This form will be sent to your Academic Advisor for approval. If approved, it will then be processed within 5-7 business days. This form is not intended for changing a student’s level (i.e. Associates to a Bachelor’s or Bachelor’s to Master’s) or adding a pathway that is not offered under the student’s eligible catalog.
