Change of Minor Request

Complete this form to request adding a minor or changing your minor. This form will be sent to your Academic Advisor for approval. If approved, it will then be processed within 5-7 business days.

This form is not intended for changing a student’s level (i.e. Associates to a Bachelor’s or Bachelor’s to Master’s) or adding a pathway that is not offered under the student’s eligible catalog.

Note: International students are required to gain approval from the Office of International Students and Scholars (OISS) to ensure changes do not jeopardize their F-1 or J-1 status.
