On-Campus Health Services Fee Opt-In Agreement (2022-2023)

Effective February 3, 2020, this form is inactive for spring semester.

Students who want to sign up after February 3, 2020 may go to the Health Services Office at Aloha Tower Marketplace, Ste. 1315, and inquire if they will allow students to sign up for the health services ($100 per semester fee) now that the spring deadline has passed. Questions? Call 808-544-9361.
