Part A: From the Student 1. Please fill out the form below completely and correctly. Your request should be submitted to your Instructor a minimum of ONE (1) WEEK PRIOR to your desired exam date. 2. You are encouraged to submit FINAL EXAM REQUEST(S) early. To ensure proper accommodations are provided, we expect to receive your request no less than TWO (2) WEEKS PRIOR to the WEEK OF FINALS. 3. Once this form is submitted, you will receive an automatic copy of this request to your HPU email. 4.
