On-Campus Health Services Fee Opt-In Agreement (2021-2022)

For the academic year 2021-2022, continuing (full-time) undergraduate and graduate students who enrolled prior to Fall 2021 and new and continuing part-time students are eligible to receive on-campus health services, which is provided by a third party provider, SP Health Clinic, at the Health Services Office (Aloha Tower Marketplace, Ste. 1315). This non-refundable Health Center Fee is $130 per semester for undergraduates and $50 for graduate students for unlimited visits while the Health Services Office is open 24 hours a week during the fall 2021 and spring 2022 semesters. There is no co-pay at the time of visit, and health insurance is not required to be seen.

Any other conditions or diagnosis falling outside the scope of these common illnesses will be subject to referral to an off-campus medical provider, based on the clinical staff’s judgement.
