Student Attestation for University Student Travel Exemption Program (Spring 2021)

I hereby attest as follows:

1. I have read, understand, and hereby acknowledge the attached “COVID-19 Modified Quarantine Procedures for University Students,” dated July 13, 2020 (“Procedures”). I understand that a violation of the Procedures may subject me to a fine of up to $5,000 and/or up to a year in prison, in addition to university sanctions.

2. I have read, understand, and hereby acknowledge the HPU Code of Student Conduct. I understand that a violation of the Code of Student Conduct, including all safety and health policies, may subject me to discipline as set forth in the Code.

3. I understand that I will not travel if I have a fever or any of the potential symptoms of COVID-19 (as listed in the Mandatory State of Hawaii Travel and Health Form, https://health.hawaii.gov/travel/traveler-health-form/) in the 14 days before my trip to Hawaii.

4. I understand that I will not travel if I have had any exposure to any known COVID-19 cases in the 14 days before my trip to Hawaii.

5. I will meet all of the conditions required in the Procedures, including:

a. I understand, and will practice, the hygiene measures described in the Procedures for at least the 14 days before my trip to Hawaii and throughout the modified quarantine period.

b. I will wear face coverings and comply with all social-distancing rules in the November 23, 2020 Sixteenth Supplementary Proclamation (“Proclamation”): https://governor.hawaii.gov/wp-content/uploads/2020/11/2011098-ATG_Sixteenth-Proclamation-Related-to-the-COVID-19-Emergency-distribution-signed.pdf.

c. I will travel directly from the Honolulu airport to my on-island residence by way of a university-provided shuttle or by means other than public transportation or pooled rides (e.g. shared uber, shared taxi). I understand that the only stop I may make from the Honolulu airport to my on-island residence would be to obtain a City-provided, State-approved COVID-19 test at a community health center. I understand that I am prohibited from renting a vehicle.

d. I will not leave my on-island residence for the lesser of 14 days from arrival or upon receipt of university notice of travel exemption approval; unless it is to seek medical attention or to obtain a State-approved, COVID-19 test.

e. If I am approved for modified quarantine, I may not leave my on-island residence except as needed to seek medical attention, obtain a State-approved COVID-19 diagnostic test, or attend official university activities including, but not limited to, orientations, safety trainings, university-provided meal services, and classes. If I experience any symptoms of illness, I will not break self-quarantine unless to seek medical treatment.

f. If I am approved for modified quarantine, I will receive an approval notice from the university. I will carry a copy of the notice during the 14-day modified quarantine period.

g. I must complete the university’s health screening procedures, including its daily symptom questionnaire, which mandates daily temperature checking (temperatures not to equal or exceed 100.4 degrees) and certification of modified quarantine compliance. I understand that I must respond to the university’s wellness inquiries as failure to respond to a single inquiry will result in the rescission of my travel exemption approval.

h. If I receive results indicating a positive COVID-19 diagnosis, I will remain in my on-island residence and immediately seek medical care and may be required to quarantine until otherwise instructed by the university or Hawaii Department of Health.

i. If I receive results indicating a positive COVID-19 diagnosis, I will immediately inform the university of my results.
